Individual and Sponsorship registrations will be on a first come, first served basis.

Registration deadline is March 23, 2012.  

Because of this, we must ask that payment be received in full at time of registration.

All available spots fill up quickly, so be sure to send in your registration well before the deadline. 

For sponsorships, if your choice is not available, you will be notified to change your selection.

To register, download a Golf Participant Agreement Form by clicking on the link below and saving it to your computer. After completing the form, you can submit it via e-mail to the address below (it is on the form as well) or mail it along with a check to our office (it is also on the form) before March 23, 2012.

To bid on the Celebrity Auction, clink on the link below and save the form to your computer. After completing the form, submit it via e-mail to the address below (it is on the form as well). New bids will be updated on the site weekly. Please check the Auction Status page to see if you need to rebid. You can rebid using the above format.
